Welcome to the ECLC Parent Portal!
Please bookmark this page and refer back to it throughout the year. It will be your guide to everything you need to know at the ECLC.Take some time to review all the information below. There are several forms that need to be filled out and submitted by September 1st. There is also some important information about challah, pizza, policies and procedures, important dates and more! If you have any questions, don’t hesitate to contact our office.
Mandatory Forms
These forms are mandatory and must be completed for each child. All forms must be completed by September 1st in order for your child(ren) to start school. Please note that medical forms are valid for only ONE year. If you submitted a form for Camp Chaverim, it can be transferred over to the school year.
Please email your completed medical forms to eclc@tinyc.org. We cannot accept a picture of your child’s health forms in the body of the email, please provide us with a hard copy or a scanned version as an attachment.
Health Form
Cumulative Health record: for new students only – and can be filled out by a parent
Individual Allergy and Anaphylaxis Emergency Plan: this form is to be completed for any child with a known allergy.
Student COVID Vaccination: for students who have received a COVID vaccination. You will need to update us with any subsequent shots.
Student Information Form: includes family and medical information, authorized pickup list, permission slip, and Parents Association dues.
Other Forms + Information
CHALLAH: A delicious way to bring Shabbat home with you! Sign up to get a regular, egg free, chocolate chip, or whole wheat challah on Fridays.
PIZZA WEDNESDAYS: For Red, Orange, and Green Room only: Take a break from packing lunch and sign up for pizza delivery!
Enrichment
We are excited to offer you a fantastic array of after-school programs and activities. These classes are a great way to supplement your child’s school day in a safe and familiar environment.
As with all of our after school programs, registration is first come, first served and there is a minimum and a maximum so please register early to ensure your choices.Policy and Procedures
